For the Attention of the Manager Kosem Coffee Roasters Dear Manager, Formal Complaint Regarding Serious Safety Failures and Distressing Incident I am writing to express my profound disgust and concern following the extremely dangerous and distressing experience my family and I had during our visit to your premises on 18-11-2025. What occurred represents a severe breach of basic health and safety standards, and it is only due to luck and quick action that the situation did not result in a tragic outcome. 1. Dangerous Road Condition With No Warning Signs Upon arrival, we encountered a massive hole in the road directly outside the premises. This hole was filled with water and appeared to be nothing more than a puddle, making it impossible for anyone to identify the serious hazard beneath. There were no signs, cones, barriers, or warnings of any kind to inform visitors of this danger. 2. Our Two-Year-Old Daughter Fell Through a Broken Bridge Even more alarming was the incident that occurred while our daughter, aged two, was walking along the pathway to view the scenery. As she crossed a small bridge on the walkway, the structure gave way beneath her. The planks were loose, inadequately secured, and dangerously covered in a way that concealed the hazard. The image attached is from after the incident. She fell straight through into the lake, becoming trapped under the bridge. She sustained cuts to her head from hitting the structure on the way down. Given her age, size, and inability to free herself, this could have easily resulted in drowning. I was only able to pull her out because I was immediately beside her. It is terrifying to consider what could have happened otherwise. No Warning Signs or Clear Guidance There were no signs whatsoever to indicate the bridge was unsafe, broken, or restricted. We simply followed the pathway as any visitor naturally would. When I raised this with your staff, I was informed, “You’re not supposed to walk here; this is for staff.” That response was both unacceptable and alarming. How is any member of the public supposed to know this without signage? Your staff acknowledged that signs “should have been there” and offered nothing more than an apology. This demonstrates a concerning lack of understanding of the severity of what occurred. This was not a minor inconvenience—this was a life-threatening incident involving a young child. Other Visitors Were Also Unaware. As we left, we encountered another family heading in the same direction. They were equally unaware of the hazards. We felt obliged to warn them ourselves, as no one from your staff appeared to take immediate action to prevent others from being put at serious risk. Potential Further Action As a family, we are currently discussing whether to pursue further action, as we believe this represents gross negligence. I sincerely hope there is CCTV footage documenting this incident, and I expect this matter to be investigated urgently and thoroughly. Requested Actions I am requesting the following: • A full written explanation of how such serious safety hazards were allowed to remain unaddressed. • Confirmation of what immediate steps you have taken to secure the area and protect the public. • Assurance that proper signage, barriers, and repairs will be implemented without delay. • Confirmation that CCTV relating to the incident will be preserved. This incident has caused significant distress to our family. We expect this complaint to be taken seriously and look forward to your prompt response. Yours sincerely, Food: 1 Service: 1 Atmosphere: 1

ก่อนอื่นทางร้านต้องขออภัยเป็นอย่างสูง ทางร้านขอชี้แจง ดังนี้ 1.ตรงจุดเกิดเหตุ ทางร้านไม่ทราบมาก่อนว่ามีการผุพัง เลยไม่ได้ติดป้ายประกาศหรือแจ้งเตือน ทางร้านจะรีบดำเนินการแก้ไขโดยเร็วที่สุด เพื่อความปลอดภัยของลูกค้า 2.ถนนทางเข้าร้าน ได้มีการแจ้งหน่วยงานที่เกี่ยวข้องไปแล้ว แต่ยังไม่ได้รับการแก้ไข อย่างไรก็ตาม ทางร้านยินดีชดใช้เยียวยาค่ารักษาพยาบาลให้ ด้วยความนับถือ
